Inclusion Criteria:
Within 16 hours of meeting Sepsis-3 definition:
- Vasopressor requirement to maintain MAP ≥65 mmHg.
- Lactate >2 mmol/L.
- Suspected/ confirmed infection.
Exclusion Criteria:
Any of the following:
- Already on or planned for RRT.
- Goals-of-care limitations.
- Active bleeding or on ECMO\IABP.
- Cirrhosis, severe TR, or anatomical barriers to ultrasound (e.g.: nonremovable surgical dressings).
- Post-cardiac arrest or liver\kidney transplant patients.
- Severe metabolic/electrolyte derangements (e.g.: K <3, Na >155)
- Inability to measure fluid balance accurately.
